REQUIREMENTS FOR SCHOOL ENTRY

When you come to register your child for school, remember to bring a few items with you concerning your child.

  • Birth Certificate
  • Immunization Record (click here to see the NJ State requirements).
  • History of allergies.
  • Information on previous injuries, chronic illnesses or conditions.
  • Information on medications your child takes, or might need at school. (we'll give you the forms to complete or click here to see & download medication forms).
  • Family health history as it relates to the child.
  • Emergency contact information -
    • we ask that you supply information on how to reach:
  • parent(s)/guardian(s) during school time &
  • contact information for two (2) additional people, who may be contacted to take charge of your child in the event of injury or illness, when you can not be reached.

Children enrolling in a school for the first time will need to have a medical exam (physical) done by their personal health care provider and supply a report to the school health office.


"Health screening" means procedures designed to detect previously unrecognized conditions as early as possible in order to provide early intervention and remediation and to limit potential disability or negative impact on scholastic performance. (Programs to Support Student Development, N.J.A.C. 6A:16)

HEALTH REQUIREMENTS & SCREENINGS FOR NEWLY ENROLLING STUDENTS

Health History Parents/guardians complete the form which provides health background information about the child's health.
Medical Exams (Physicals) Physicals are needed by family's health care provider prior to school entry and on a regular basis throughout the school age years. 
Exam reports are requested for:
     those starting school for the first time, 
     4th and 
     7th graders in this district. 
Forms are available from the school nurse.  
Immunizations NJ requires that children comply with the state immunization guidelines for school attendance. Immunization records are evaluated & recommendations are made at the time of registration. Atlantic County Health Department in Northfield provides free or low fee immunizations.
Communicable Diseases Children exhibiting signs/symptoms of communicable disease are not admitted until symptom free.
Head Lice Head lice checks are done prior to school admission. Children with lice/nits are not admitted until nit-free.
Tuberculosis (TB)
Mantoux PPD Skin Testing
Mantoux TB PPD tests are required (NJ State Health Guidelines) on all children entering from outside the state of NJ & from NJ Department of Health & Senior Services (NJDHSS) specified areas within the state. Learn more about TB tests: click here.

HEALTH SCREENINGS DONE FOR ALL STUDENTS

SCREENING

ASSESSED

Growth Height & weight are checked in a growth screening. A weight gain of 4 - 5 lbs. is the norm for each inch of height gain. If a child isn't growing according to expectation, a referral is sent home. This might include failure to gain in height &/or weight or gain in height &/or weight that is significantly greater than expectation. It is important, at this age, to stress the norm of weight gain to height gain so that the children understand that weight gain of 4 - 5 lbs. per inch of height gain is the norm during the growing years.
Vision Far vision, near vision, color vision, & lateral & vertical eye muscle balance are assessed in vision screening.   Vision is checked periodically to assure that vision problems are detected & treated timely so that learning is not impaired. The Titmus Eye Machine is used. Referrals are sent home for any findings that are not within normal limits.
Hearing Audiometric screening assesses hearing. If a child fails a hearing screening, a second screening is done after 4 - 6 weeks. Colds & active allergies can be responsible for screening failures. If the child fails the 2nd screening, a referral is sent home recommending further medical evaluation.
Scoliosis Spine curve is checked annually for all children 10 yrs. old and older per NJ State Health Guidelines. If a curvature of 7% or more detected, a referral is sent home. Often there may be minor deviations that occur during periods of active growth or growth spurts. These usually correct by the next annual screening. If deviations in excess of 7% exist, orthopedic consultation is recommended.
Communicable Disease If a communicable disease is reported in one of our children, the child's classmates will be observed for possible symptoms. Parents are informed of the presence of communicable diseases within the grade or school as warranted.
Head Lice Periodic head checks are done during each school year to prevent any head lice outbreak. It is recommended that parents check their children before the start of each new school year & on a regular basis during the year as well. Notice is sent home if cases of head lice are found in the grade level. Any child found to have head lice &/or nits (head lice eggs) is excluded from school until nit-free per Brigantine Board of Education policy - ALL NITS MUST BE REMOVED TO RETURN TO SCHOOL. Return to class is permitted after the child is checked and cleared by the school nurse.

MEDICAL EXAMINATIONS (PHYSICALS)

Sports Physicals (Pre-participation physical exams) are required for participation in school sports & must be done prior to first practice or tryouts.  
Sport Pre-participation Physical & Health History Questionnaire Forms , are to be completed by child's health-care provider and returned to the school nurse prior to the first practice session.  Forms are available here.  The state of NJ Department of Education requests that sport physicals be done by each child's family health provider (N.J.A.C. 6A:16-2.2).  If there is no health insurance or there is inability to afford cost of a medical exam, please notify the school nurse - 266-3603.
For the uninsured, health insurance is available through  NJ Family Care.
Children between the ages of 13 and 19 years may use the services of the Atlantic City Teen Center, which is located in the Atlantic City High School Complex.  Call 345-8336 for an appointment.

A sport physical exam is valid for one (1) year.  If it was done more than 60 days prior to first practice, then a health history questionnaire as update is required to document that there are/are not any changes in health status since the sport physical was done.
Regular Physicals (Medical exams) are encouraged during the School Years.  All students, entering school for the first time, are required to have a physical by their personal health care provide, and a report given to the school health office.  
In accordance with N.J. State Health Guidelines, beginning in the school year 2001-2001, all 4th graders, in elementary school, and all 7th graders in middle school are to have a physical performed by their personal health care provider and a report sent to the school health office.  Forms are provided by the school health office.  Call if you need one - 266-3603.  For the uninsured, health insurance is available through  NJ Family Care.
Special Request The Child Study Team may request a medical exam, with parent/guardian permission, as part of the student evaluation process.  The exams are done by personal health care provider and report sent to school health office.  If the family has no health insurance for the child, the school will be responsible to assure that an exam can be completed.  For the uninsured, health insurance is available through  NJ Family Care.
